General
To qualify as a sperm donor, you need to be aged 21–39, in good overall health, able to pass a background check, willing to commit to 1–3 donations per week, and living near one of our locations - Indianapolis,IN or Nashville, TN. Meeting these requirements is the first step; our screening process covers the rest.
Complete our short online application — it takes only 3 minutes and is completely confidential. We'll assess your eligibility, and if you qualify, a donor coordinator will reach out within 48 hours to guide you through the next steps.
Approval typically takes 2–4 weeks from application to acceptance, depending on scheduling and application volume. The process includes your online application, a semen analysis, medical screening with a physical evaluation, and a background check. Our team guides you through each step and works around your availability.

Yes. All our donors complete a clinical evaluation with a qualified, licensed mental health professional. The session assesses emotional and psychological readiness and confirms you understand the donation process and its implications. It's designed to protect both you and the families you'll help, and to verify that your motivation and commitment align with Cryomate's standards.
At Cryomate, we do consider specific height and weight criteria for sperm donors to facilitate potential matches with recipients who may have preferences regarding physical characteristics. However, these criteria may vary and are subject to adjustment based on the evolving needs of our program and the preferences of our clients. Currently, we encourage donors who are 5'7" or taller to apply.
The most common disqualifying factors are underlying medical conditions, genetic disorders, and not meeting FDA donor eligibility criteria — which involve a physical exam, medical history questionnaire, and infectious disease testing at an FDA-approved lab. Height, weight, and the psychological evaluation are also considered.
Acceptance criteria differ between cryobanks. If the reason for your disqualification was medical, you likely won't qualify with us either. If the reason for rejection was related to the quality of your sperm, we recommend that you continue with your application.
Tell your friends about the program and ask them to apply online. Once a friend is accepted and gives us your first and last name, you'll receive your $500 referral bonus.
Sperm donation is a safe, low-risk process — collection is straightforward and involves no surgery or invasive procedures. As a donor you also benefit from comprehensive medical screening at no cost, so you'll have an ongoing picture of your own health throughout the program, all while helping individuals and couples build their families.
Sperm Donor Compensation
Active donors can get paid up to $1,200 per month via ACH transfer once approved for the donation program, plus a $500 bonus for each friend you refer who's accepted. Your donor recruitment coordinator will walk through the full compensation details during your interview.
Yes. Sperm donation payments are classified as self-employment income for tax purposes. You'll receive a 1099 form, and you can report your earnings and related expenses on your tax return. Your donor recruitment coordinator can provide further details.
Donor Commitment
Donors can make 1–3 donations per week. We ask for a commitment of at least 6 months, visiting our office at least once a week — and many donors choose to continue beyond this initial period. We collaborate with all donors to accommodate their schedules, ensuring flexibility for donation times.
Safeguarding & Regulation
Yes. We prioritize your privacy. Your application information is transmitted securely, and every donor receives a unique donor ID that protects your identity throughout the program. If you choose open-ID donation, your information may be shared with donor-conceived offspring once they turn 18 — and only after Cryomate confirms with you first.
No. As a donor, you’ll sign an agreement that waives all parental rights and responsibilities for children conceived from your sperm.
Online DNA testing can potentially impact the anonymity of sperm donors. If you or your relatives undergo DNA testing on platforms like Ancestry or 23andMe and choose to share connections through their database, there's a risk of being identified by others. It's important for sperm donors to be aware of this and consider privacy preferences when using such services.
